International Health Forms for Dogs

As of July 31, 2025 the CDC has changed the import and export requirements for bringing dogs in and out of the USA. These changes will especially affect you if you are coming back from a country that is considered to be High Risk for Rabies.


Please call us a minimum of 60 days before you plan to travel so we can do the required research, vaccinations, microchipping and fill out the appropriate forms in a timely manner so you can leave on your planned date.

The USDA and CDC websites have some of the most current information to start your research on what will be required for your dog to travel with you. The minimum requirements are a microchip and a current Rabies vaccination given after the microchip was implanted.
